Biliary intraepithelial neoplasia: a case with benign biliary stricture

چکیده
Intrahepatic and extrahepatic cholangiocarcinomas develops through a multistep carcinogenesis sequence and they are preceded by precursor lesions. Precursors of malignant neoplasm are identified in various organs including uterine cervix, gastrointestinal tract and breast. Identification of these precursor lesions are important in that they need minimal treatment or they are good indicator of disease progression or recurrence. In the intrahepatic and extrahepatic biliary tract, precursor lesions of malignancy are defined as follows; biliary intraepithelial neoplasia (BilIN), intraductal papillary neoplasm, mucinous cystic neoplasm, and adenoma. BilIN is characterized by atypical epithelial cells with multi-layering of nuclei and micropapillary projections into the lumen of bile duct. In contrast to intraductal papillary neoplasm which is characterized by papillary proliferation of epithelial cells with fibrovascular stroma and mucin production, BilIN tend to be a microscopic lesion. Mucinous cystic neoplasm is usually cystic lesions and has distinguishing clinicopathologic findings such as subepithelial stroma resembling ovarian stroma and predominant occurrence in adult female. Adenomas are commonly encountered in extrahepatic bile ducts and divided into tubular, papillary and tubulopapillary on the basis of growth pattern. CASE SUMMARY

Incidence of underlying biliary neoplasm in patients after major hepatectomy for preoperative benign hepatolithiasis
BACKGROUNDS/AIMS Despite hepatolithiasis being a risk factor for biliary neoplasm including cholangiocarcinoma, the incidence of underlying biliary neoplasm is unknown in patients with preoperative benign hepatolithiasis. The aim of this study was to evaluate the incidence of underlying biliary neoplasm in patients who underwent major hepatectomy for preoperative benign hepatolithiasis. METHO...
متن کاملSuccessful Use of Biliary Duct Balloon Dilator in Repairing Post-Surgical Esophageal Stricture in premature infant
Congenital esophageal atresia needs to be surgically corrected as soon as possible. Some will present with post surgical stricture that needs to be dilated by a balloon dilator. This procedure is difficult in premature born due to infant esophageal size and availability of equipments. We herby report the first case in a premature infant with post surgical esophageal stricture of esophageal atre...
متن کاملA guidewire-assisted biopsy technique to assist advancement through a biliary stricture to perform selective mapping biopsy.
An endoscopic transpapillary mapping biopsy is useful for evaluating intraepithelial tumor spread in bile duct cancers [1,2]; however, it is occasionally technically difficult to advance the biopsy forceps through a biliary stricture. Here, we describe the case of a patient with a hilar biliary stricture who underwent a selective mapping biopsy. The biopsy technique described is suitable where ...
متن کاملEosinophilic Cholangitis—A Challenging Diagnosis of Benign Biliary Stricture
When confronting a biliary stricture, both benign and malignant etiologies must be carefully considered as a variety of benign biliary strictures can masquerade as hilar cholangiocarcinoma (CCA). Therefore, patients could undergo a major surgery despite the possibility of a benign biliary disease. Approximately 15% to 24% of patients undergoing surgical resection for suspected biliary malignanc...
